Job Description:
At Agape Care Group, we are seeking a dedicated Medical Social
Worker to join our team of compassionate professionals in providing
hospice and palliative care throughout the Southeast. This role
involves supporting patients and their families by addressing
psychosocial, socio-economic, and emotional needs during
challenging times. Our ideal candidate is someone who believes in
preserving dignity and enhancing the quality of life for our
patients, embodying our mission to serve with love, providing
personalized and thoughtful care. Assess the psychosocial status of
patients and families to support overall healthcare plans. Provide
counseling and crisis intervention for patients and their families.
Develop and implement individualized care plans in collaboration
with an interdisciplinary healthcare team. Facilitate connections
to appropriate community resources to assist in recovery,
rehabilitation, or end-of-life care. Advocate for patient rights
and necessary services based on continual assessments. Document
patient cases including assessments, progress, and interventions.
Participate in team meetings sharing insights from a social work
perspective. Strong ability to empathize and communicate with
individuals facing health crises. Proficient in case management
software and medical recordkeeping. Knowledge of healthcare policy,
along with local and federal regulations related to social work.
Crisis intervention skills and effective coping mechanism
strategies. Medical Insurance Dental Insurance Vision Insurance
Life Insurance 401k Current licensure as a Licensed Clinical Social
Worker (LCSW) in the state of employment. Certification in Hospice
and Palliative Social Work (CHP-SW) preferred.
