be registered with the Board before the person can provide services

The Clinician QMHP-C will work as a member of the integrated care
team to ensure the implementation of an individualized treatment
plan to individuals receiving services through residential
placement and discharge from the Blue Ridge Regional jail into the
Required Education and Experience:
- Bachelor's degree from an accredited college/university in an
applicable human services field.
- A minimum of one year of documented clinical experience or an
equivalent combination of education and experience that qualifies
the incumbent as a QMHP-C or QMHP-A.
The Forensic Clinician QMHP-C will report to the Clinical
Coordinator – Juvenile Justice, and will be responsible for the
- CLINICAL CASE MANAGEMENT: Implements the individualized
treatment plan through appropriate linkages, referrals,
coordination and follow-up to needed services and supports,
including referral and linkages to long term services and supports.
Ensure appropriate care is provided through coordination of Horizon
and external resources. Monitors, evaluates, and records individual
progress according to measurable goals described in treatment and
care plan. Identifies and documents environmental impediments to
the individual's progress through interviews and observations.
- ASSESSMENTS/PLANNING: Conducts ongoing comprehensive
assessments of individual's strengths, abilities, preferences, and
needs and then modifies treatment plans as necessary to comply with
changes in the individual's status. Participate in integrated
treatment teams; team meetings; treatment planning; and information

- ADVOCATE/COLLABORATION: Advocates for individuals and families
by providing necessary resources to support the individual's
behavioral health, primary care, and health/wellness needs.

care is provided through coordination of Horizon and external
- EDUCATION/COUNSELING: Conducts support groups and counsels
family members to assist them in understanding, dealing with, and
supporting the individual. Plans and conducts programs to combat
social problems, prevent substance use, and/or improve community
health and counseling services.
- MISCELLANEOUS RESPONSIBILITIES: Responsible for familiarizing
self with training requirements as assigned on Horizon Learning
Management System and completing within assigned deadlines.
Responsible to access Horizon's LMS and accept individual
responsibility to complete all trainings on time. Completes other
duties and projects as assigned. Committee/Meeting participation:
Actively participates, solution-oriented, and focuses on
constructive problem solving, as assigned. Facilitate trainings for
Horizon staff and/or community members as requested.

Related Knowledge, Skills and Abilities:
Knowledge of behavior modification; Cognitive Behavioral Therapy
(CBT); emergency interventions; family, group, and individual
therapies; psychosocial education; risk assessments; sex offender
treatment; substance abuse interventions; etc. Skill to communicate
effectively in order to facilitate treatment and relay information
to others as part of an interdisciplinary team. Skill to perform
clinical interviews in order to obtain information and to formulate
treatment plans. Ability to drive and must possess a valid driver's
license. May be called upon to travel to any Horizon worksite;
transport clients, provide services to client @other locations.
Regular attendance at trainings and Horizon meetings may require
travel to a variety of Horizon sites.
Willing and able to work modified schedules (weekends, holiday,
etc.) as required.
