Heart Havens seeks a caring Lead Direct Support Professional to
work at our home in Lynchburg, Virginia who embrace our mission of

This is a Full Time position working Sunday - Wednesday 9am - 7
The Lead Direct Support Professional (LDSP) is responsible for
supporting the House Manager in the operation of the residential
program and engaging in the Direct Care of the residents in the
home. The LDSP is expected to serve as a role model to staff
members by demonstrating person-centered skills, values and
attitudes. Passion for and the desire to work with people with
developmental disabilities is key to success in this position.

Minimum of one year of experience in a residential group home
setting, or an equivalent combination of experience in the care,
training, and habilitation of individuals with developmental

documentation, and communicate across departments.

-Ability to organize schedule and implement processes to
effectively manage job duties.
-Ability to follow the policies and procedures of Heart Havens to
maintain compliance with state and federal regulations.
-Ability to observe, record and report residents' behavior,
attitude and physical condition.
-Ability to perform First-Aid, Behavior Management interventions,
and support residents with Activities of Daily Living.
-Must have sufficient computer skills in Microsoft Word and
-Must maintain required certifications: TOVA, Medication
Management, and First Aid/CPR/AED
-Engages in the Direct Care of residents to include medication
administration, support with Activities of Daily Living, and
encouraging engagement in meaningful activities at home and in the
-Supports the House Manager with the daily operation of the
residential program by completing routine quality assurance tasks,
assisting with maintaining program files, and other tasks as
-Provides leadership to the program, and in the absence of the
House Manager, serves as the point of contact for staff members and
the Program Director.
-Provides emergency schedule coverage as needed, and resolves
scheduling issues in the home when the House Manager is
-Identifies and responds to changing health and behavioral needs of
the residents by reporting concerns to the House Manager and/or
-Attends resident's routine medical appointments as scheduled by
the House Manager, documents care outcomes, and reports critical
information to team members.
-Reviews and audits daily documentation of resident checklists and
daily cleanliness of the home.
-Conducts weekly program medication audit ensuring there is an
adequate supply of medication on hand, electronic medication
records are accurate, and pharmacy records are up-to-date and filed
-Conducts weekly ISP Data report ensuring progress notes are
completed for each shift for each resident, alerting staff members
of missing notes, and providing staff members with quality
improvement guidance on content.
-Completes meal menus that incorporate the preferences and dietary
needs of the residents, and ensures menu changes are documented by
-Schedules and facilitates monthly resident meeting ensuring the
opportunity for residents to express choices, share preferences,
and communicate their needs.
-Completes monthly resident activity calendar incorporating
research of local activities and events, balance of individual and
group activities, and preferences of the residents.
-Follows budgetary guidelines set forth by the COO when using Heart
-Completes all steps of grocery shopping for the home including
making the shopping list and purchasing the items.
-Participates in on-call rotation with House Manager as approved by
-Performs other job-related duties as assigned.
