Behavioral Health Navigator - REACH - On-call Location: Lynchburg VA 24501 Hours: On-call Position Summary: When scheduled, provide after-hours response (6:00 pm - 8:00 am) to children and youth diagnosed with ID/DD, who are experiencing a crisis episode. When scheduled for days in which Horizon Behavioral Health is closed, or on observed Holidays, requirement includes coverage 24-hours instead of evening coverage only. Meet the requirement of a 2-hour response, per DBHDS. Provide immediate crisis supports to children/youth and family. Assist ES and/or ES departments by providing additional supports for child/youth and family during pre-screening process. Provide immediate crisis supports to children/youth and family in their home setting, if deemed appropriate and safe. Complete all required documentation. Communicate effectively and efficiently with full-time Navigators, as well as other providers for linking of services. Required Education and Experience: Bachelor's degree from an accredited college/university in an applicable human services field (including social work, counseling, behavioral science, psychology, or other DMAS approved human services and related fields degrees). One year of relevant clinical experience or an equivalent combination of education and experience that qualifies the candidate as a QMHP-C. Must have at least QMHP-C to fulfill job requirements. LMHP-E and LMHP are preferred. Related Knowledge, Skills and Abilities: ---Knowledge of behavior modification; Cognitive Behavioral Therapy (CBT); emergency interventions; family, group, and individual therapies;, psychosocial education; risk assessments; sex offender treatment; substance abuse interventions; etc. ---Knowledge of community mental health and public health in order to provide advocacy and enhance an individual's treatment and access to community resources. ---Knowledge of the principles, procedures, techniques, trends, and literature related to mental health, intellectual disabilities and substance abuse. ---Knowledge of psychiatric/medical medications/treatments. ---Skill to analyze situations accurately and develop appropriate responses or techniques. ---Skill to communicate effectively in order to facilitate treatment and relay information to others as part of an interdisciplinary team. ---Skill to perform clinical interviews in order to obtain information and to formulate appropriate treatment plans. ---Skill to develop reports, create presentations, and communicate findings to both internal and external audiences. ---Skill to use database systems and software to efficiently and effectively meet programming needs. ---Willing and available to work modified schedules (weekends, holiday, etc.) as required. ---Ability to drive and must possess a valid driver's license. May be called upon to travel to any Horizon worksite; transport clients, provide services to client other locations. Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites; ---Ability to participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed. ---Ability to develop and maintain therapeutic relationships. ---Ability to promote positive, collaborative, and professional working relations among co-workers and representatives in the community. ---Ability to record data and complete documentation within established expectations and by communicated deadlines. ---Ability to respond effectively to changes in the work environment in a positive and professional manner. ---Ability to work in and contribute to a collaborative team environment.
