This role offers the opportunity to work in both facility-based and home-based settings, using advanced protocols and practices to support patient outcomes in 42 states where we operate. As a key healthcare team member, you will collaborate with multidisciplinary teams to offer treatments and daily care tailored to the needs of our patients. Provide daily nursing care and services in accordance with each patient’s plan of care. Support diagnostic procedures, assisted assessment reviews, and documentation. Administer medications and record all entries accurately in the patient’s medical record. Collaborate effectively with a multidisciplinary health care team to optimize patient care. Communicate cooperatively and constructively with patients, families, and other health care professionals. Ensure adherence to safety, healthcare regulations, and policies and procedures. Strong interpersonal and communication skills Proficient in nursing practices, protocols, and patient-centered care Ability to multitask in a dynamic, integrated healthcare environment Knowledge of electronic medical record systems Medical Insurance Dental Insurance Vision Insurance 401k Current LPN or LVN licensure in the state of practice CPR certified
